We are not a medical practice, pharmacy, dispensary, or healthcare provider. We are not affiliated with, endorsed by, or acting on behalf of any government agency, including the Illinois Department of Public Health (IDPH).
All appointment scheduling services are facilitated through Acuity Scheduling, an independent third-party booking platform. All clinical services — including evaluations and physician certification decisions — are provided solely by independent IDPH-certified Illinois physicians who are not employees, agents, or representatives of this Site.
When you book an appointment through this Site, you are scheduling with an independent, third-party Illinois-licensed physician certified by IDPH to participate in the Medical Cannabis Patient Program. These physicians are solely responsible for:
We do not supervise, direct, credential, control, or employ any physician. No provider–patient relationship exists between you and PeoriaMarijuana.Clinic. Any such relationship exists only between you and the independent physician you choose to book with.
We do not guarantee that you will receive a written certification, that IDPH will approve your registry card application, that an appointment will be available at a particular time or price, or that a physician will accept you as a patient. Certification decisions are made solely by the independent physician. Registry card approval decisions are made solely by IDPH.
Use of this Site is intended for adults who are 18 years of age or older and who are residents of the State of Illinois with a qualifying condition on the IDPH debilitating conditions list. You must hold a valid Illinois government-issued ID and acceptable proof of Illinois residency to apply for a registry card. Illinois does not accept out-of-state medical cannabis cards. Consult IDPH's Medical Cannabis Patient Program page for current eligibility rules.
Appointment booking is facilitated through Acuity Scheduling. Payment may be made by major credit card at the time of booking.
Refund eligibility, cancellation rules, and no-show policies are governed by the policy disclosed at the time of booking and/or the independent physician's posted policy. PeoriaMarijuana.Clinic does not control refund decisions and is not responsible for issuing refunds unless explicitly stated otherwise in writing.
If you do not receive a written certification following your appointment, you may be eligible for a refund of the consultation fee in accordance with the booking platform's refund policy. The IDPH state government registry card fee — $50 (1 year), $100 (2 years), or $125 (3 years) — is paid directly to the Illinois Department of Public Health through ilogin.illinois.gov and is non-refundable; refund eligibility for the IDPH fee is governed entirely by IDPH's own policies.
